I consent to my card being charged for my membership on the lith & or 25th of the month 1. or both if I choose 2x or 4x month. If I no-show/cancel my appointment less than 24 hours before the service time I will lose that service for the month with no rebook. I understand Perry Massage has limited hours and I will need to set my own appointments to use 2. my membership. I know that refunds are not accepted if I choose to not redeem my services and I can roll over services month to month until the end of the current year, which will cause my services to expire unless used by me or my person I'm sharing with. Once I cancel my membership, all my roll-over unused services will become void unless used in 30 day notice window.
